    Athletics Paperwork

    Paperwork required for DSISD student athletes should be submitted electronically for the 2020-21 school year.  High School students may begin submitting their electronic UIL forms as of Wednesday, August 5, 2020NOTE:  If anyone starts submitting prior to August 1, 2020 you must re-submit to electronically sign current year forms.

    Middle School 2020-21 electronic UIL forms should not be submitted until after the first day of school, August 18, 2020. 


    Physical Form

    (for all Athletes, Band, Hi-stepper and Cheer students)
    UIL Preparticipation Physical Evaluation Form 

    NOTE:  Do not download the student physical in RMA, this section will show as "Incomplete" until the Athletic Trainers verify receipt of current year physical.  Returning students are allowed to use their 2019-20 Physical form on file at this time, only incoming 7th grade and new to the District students are required to furnish a new Physical form for the 2020-21 school year.

    HS athletes should turn in their completed UIL physical form to the Athletic Trainer at the Athletics Office in the HS Stadium Field House.

    HS Band, hi-stepper and cheer students may turn their forms in to their DSHS sponsors.

    MS athletes should turn in their physical forms to their campus coach before the first day of practice. 



    Every student who participates in Middle School or High School Athletics, HS Band, Hi-Steppers or HS Cheerleading are required to pay a participation fee.  For the 2020-21 school year, this fee is being reduced in recognition of the economic impact and uncertainty resulting from the pandemic.  The $125 high school fee is being reduced to $75, and the $50 middle school fee is being reduced to $30. 

    Middle School fee:      $ 30
    High School fee:         $ 75

    Online participation fee payments for DSHS students will be accepted thru Skyward Family Access (preferred method of payment).  Participation fee must be paid by the Monday before the first game of student's sport.  Only one fee per school year regardless of multiple sports. 

    DSMS and SSMS online payments will be accepted thru Skyward Family Access after school starts, parents will have until September 30, 2020 to submit payment for their fee.
