  • Athletics Paperwork

    Paperwork required for DSISD student athletes can be submitted electronically, the 2018-19 High School forms are ready to submit.  Click HERE to get started completing Register My Athlete electronic UIL forms. 

    NOTE:  Do not download the student physical, this section will show as "Incomplete" until the Athletic Trainers verify receipt of current year physical.  See RMA Instructions for Parents for assistance, or contact the DSISD Athletics Office at (512)858-3037.

    Middle School 2018-19 electronic UIL forms should be completed beginning the first day of school. 

     

    Physical Form

    (for all Athletes, Band, Hi-stepper and Cheer students)
    Physical Evaluation Form (Required UIL Form)
    HS Athletes may turn in their completed UIL physical form to the Athletic Trainer at the Athletics Office in the HS Stadium Field House.
    Band, hi-stepper and cheer students may turn their forms in to their sponsors.

    MS athletes should turn in their physical forms to their campus coach before the first day of practice. 

     

    2018-19 Student Extracurricular Participation Fee

    Every student who participates in Middle School or High School Athletics, HS Band, Hi-Steppers or HS Cheerleading are required to pay a participation fee.

    Middle School fee:      $ 50
    High School fee:         $125

    Online participation fee payments for DSHS students will be accepted thru Skyward Family Access (preferred method of payment).  Participation fee must be paid by the Monday before the first game of student's sport. 

    DSMS and SSMS online payments will be accepted thru Skyward Family Access after school starts, parents will have until September 30, 2018 to submit payment for their fee.
