Health Information for Parents
Texas State Law requires that ALL 7th graders receive the following vaccines:
- Tdap booster - 1 dose within the last 5 years
- Meningococcal vaccine - 1 dose of the quadrivalent vaccine on or after 11th birthday
For those parents who do not want their children to be vaccinated for reasons of conscience, a notarized Affidavit of Exemption must be filed with the school.
In order to have access to your child's schedule in Skyward and to start school, proof of immunization or a current, notarized affidavit must be received.
If your 6th grade student has already received the 2 vaccinations required for 7th grade, please feel free to submit proof of vaccination to the nurse now. This will be one less item you have to think about for next school year!
You may fax, mail, email, or bring the updated shot records to the school.

I earned my Bachelor of Science in Nursing from the University of Texas at Austin in 2003. After graduation, I worked at Seton Medical Center in Austin for 11 years. This is my 5th year at DSMS & I firmly believe I have the very best job on Earth. It is such a privilege to know and care for your children. I am married to a Drip native (Class of '93) & we have one daughter, Taylor, in 9th grade. In my spare time, I love to read, bake, travel, and play fantasy football.
