Athletics Paperwork
Paperwork for DSISD student athletes can be submitted electronically.  Please submit 2016-17 High School forms beginning on Saturday, August 6, 2016.  You will need the Student ID number to begin. 

Middle School 2016-17 electronic forms will be accepted beginning on Friday, August 12, 2016. 

Do not include zeros before student ID numbers.  Click HERE to get started completing Rank One electronic forms.

Athletes may turn in their forms to the Athletic Trainer at the Athletics Office in the HS Field House.
Band, hi-stepper and cheer students may turn their forms in to their sponsors.

2016-17 Student Extracurricular Participation Fee
Every student who participates in Middle School or High School Athletics, HS Band, Hi-Steppers or HS Cheerleading are required to pay a participation fee.

Middle School fee:      $ 50
High School fee:         $125

Online payments are accepted thru Skyward Family Access beginning Saturday, August 6, 2016 (preferred method of payment). If necessary, cash or check payments will be accepted at the HS Field House Athletics Office, Monday-Friday, 9:00 a.m.-4:00 p.m.  Participation fee must be paid by the Monday before the first game of student's sport.
