YOUR STUDENT'S VACCINATIONS ARE ON SKYWARD FAMILY ACCESS


When you log into Skyward Family Access, click on the "Health Information" tab in the left-hand column to see all of the vaccination information that we currently have on file for your student.  You can print a complete vaccination record right from home!


PARENTS:

ALL INCOMING 9th - 12th grade students MUST have received 2 doses of Varicella vaccine, 1 dose of Meningococcal vaccine, and a Tetanus booster (TDap) within the last 10 years. (If your child had the chicken pox disease, the Varicella vaccine is not required - provide date of illness to school nurse.)

Please check you child's immunization records to verify they are up to date before school starts August 2016.

Foreign Exchange Student Immunization Notice - We LOVE our exchange students, but please do not wait until the first day of school to present your immunizations - you will not begin classes if your immunizations do not meet Texas requirements.

Medication Permission Form - this form is for prescription medications, inhalers, epi-pens, etc.

New : High School Students (9th-12th) may carry over the counter medication if parent consent is provided to the nurse, using the form below:
